Erectile dysfunction rising among young men, doctors warn of impact on fertility

A growing number of young men in their 20s and early 30s are seeking medical help for erectile dysfunction (ED), a condition once largely associated with older age, raising concerns among fertility specialists about its impact on conception and long-term reproductive health.

Doctors say untreated ED can affect a couple’s chances of conceiving, as it interferes with the ability to achieve and maintain an erection required for sexual intercourse. Increasingly, specialists are observing cases among men under 35, many of whom delay seeking help due to embarrassment, stigma or the assumption that the problem is temporary.

Erectile dysfunction is defined as the persistent inability to achieve or maintain an erection firm enough for sexual activity. While occasional difficulty is not uncommon, frequent or ongoing symptoms may indicate an underlying medical or psychological condition.

Lifestyle, stress and medical conditions driving ED

Medical experts attribute the rise in ED cases among younger men to a combination of lifestyle factors, stress and undiagnosed health issues.

“Common causes include stress, anxiety, performance pressure, smoking, alcohol overuse, low testosterone, obesity, diabetes, hypertension, high cholesterol and poor sleep,” said Dr Parul Gupta Khanna, Fertility Specialist at Nova IVF Fertility, Vasant Vihar, Delhi.

She added that behavioural factors such as excessive pornography consumption, lack of physical activity and relationship stress can also contribute to the condition.

“In a month, nearly five out of 10 men aged between 27 and 35 report symptoms such as difficulty getting or maintaining an erection, reduced sexual desire and anxiety during sexual activity,” she said.

Doctors warn that ED may sometimes serve as an early indicator of more serious health problems.

“Erectile dysfunction can be an early warning sign of cardiovascular disease, hormonal imbalance or metabolic disorders,” Dr Khanna said. “Ignoring symptoms may delay diagnosis of these underlying conditions.”

Impact on fertility and conception

While ED does not directly affect sperm quality, fertility experts say it can significantly reduce the chances of pregnancy by limiting regular intercourse during the fertile window.

“ED affects the ability to have timed intercourse, which is essential for conception,” Dr Khanna explained. “Men may avoid sexual activity due to stress or embarrassment, or may be unable to ejaculate effectively, reducing the chances of fertilisation.”

In some cases, ED may also occur alongside low testosterone levels, which can affect sperm production and overall reproductive function.

Dr Mandavi Rai, Senior Consultant in Reproductive Medicine at Motherhood Fertility and IVF, Noida, said many young men hesitate to seek help, assuming the condition will resolve on its own.

“Men in their late 20s or early 30s often believe ED is temporary or stress-related and suffer in silence,” she said. “However, it can signal obesity, poor fitness, diabetes, hormonal imbalance or even early heart disease.”

She added that untreated ED can create emotional distress and relationship strain, further affecting fertility.

“When ED is not addressed, couples may miss the fertile window due to irregular or avoided intercourse, increasing stress and delaying pregnancy,” she said.

Early treatment and lifestyle changes improve outcomes

Doctors emphasise that erectile dysfunction is treatable, particularly when diagnosed early. Treatment approaches depend on the underlying cause and may include lifestyle changes, counselling, medication or advanced medical therapies.

“Maintaining a healthy weight, exercising regularly, reducing stress, improving sleep and quitting smoking can significantly improve sexual function,” Dr Khanna said.

Medical interventions may include hormone evaluation, medications to improve blood flow, counselling, vacuum erection devices or shockwave therapy to improve vascular health. In some cases, minimally invasive procedures or penile implants may be recommended.

“Fertility specialists select treatments based on the patient’s age, health and underlying condition,” she said.

Experts say addressing ED early not only restores sexual health but also improves the chances of conception and overall wellbeing.

“Erectile dysfunction is highly treatable when managed in time,” Dr Rai said. “Simple lifestyle changes, medical evaluation and timely treatment can restore sexual function and protect fertility.”

Doctors stress that young men should not ignore symptoms or delay seeking medical advice, as early intervention can prevent complications and improve both reproductive and long-term health outcomes.
